Healthy Living Intensive Case Manager

 

 

Hours:               Monday – Thursday

Status:              Full Time

Reports to:       Adult Housing Programs Supervisor

 

 

Purpose:

Under the direct supervision of Supervisor, this position Adult Housing Programs Supervisor, this position provides direct case management services to medically fragile, chronic homeless adults.
 

Duties, Functions, and Responsibilities:

  1. Conduct initial interview/intake and clients’ needs assessments.
  2. Provide support and arrange services to consumers moving into long term housing, to promote transition from homelessness to residential stability.
  3. Effectively explain public social services programs and assist consumers with the application and follow up, to ensure services are received.
  4. Create consumer driven treatment plans in concert with consumers and doctor recommendations that reflect the needs of the medically fragile client.
  5. Monitor client’s progress and provide follow-up services.
  6. Maintain accurate documentation of service to clients.  Enter accurate consumer date into HMIS in real time data, to reflect consumer activities and their progress.
  7. Maintain audit ready files at all times.  (Random audit checks are regularly performed)
  8. Participate in multi-agency, multidisciplinary teams to report and review consumer progress.
  9. Other duties, as may be assigned.

 

Knowledge, Skills and Abilities:

  1. Knowledge of local social service agencies and programs as they pertain to the local homeless population.
  2. Knowledge of applicable statutes, ordinances, and codes.
  3. Knowledge of social service work, interviewing/referral techniques.
  4. Experience and understanding of homeless consumer issues.
  5. Understand and follow complex instructions and procedures.  Analyze situations accurately and adopt an effective course of action.
  6. Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
  7. Effective written, verbal, and listening skills.  Independently initiates and responds to correspondence.
  8. Ability to maintain order and exercise appropriate judgment in crisis situations.
  9. Is flexible, and can quickly adjust to shifting priorities.
  10. Establish and maintain cooperative working relationships with consumers, fellow employees, other agencies, and the general public.
  11. Organize work schedule, and budget time.
  12. Understand and follow all agency policies and procedures.
  13. General computer knowledge, and the ability to use Microsoft Office software.
  14. Ability to work flexible hours, as required by consumer case load.
  15. Ability to perform accurate mathematical equations to calculate client’s rent and utility costs, and request funding to pay those bills.

 

Qualifications, Education, and/or Equivalent Experience:

  1. Bachelor’s Degree in Social Work, or High School Diploma and (2) Two Years experience, substantially equivalent to a bachelor’s degree, or combination of education and experience.
  2. Professional experience, as required by education.
  3. Must pass background check and pre-employment drug test.  Random drug screening is performed.
  4. Valid Nevada driver’s license, and clean DMV record.  Driver History Reports are required to be submitted on a quarterly basis.
