Healthy Living Intensive Case Manager
Hours: Monday – Thursday
Status: Full Time
Reports to: Adult Housing Programs Supervisor
Purpose:
Under the direct supervision of Supervisor, this position Adult Housing Programs Supervisor, this position provides direct case management services to medically fragile, chronic homeless adults.
Duties, Functions, and Responsibilities:
- Conduct initial interview/intake and clients’ needs assessments.
- Provide support and arrange services to consumers moving into long term housing, to promote transition from homelessness to residential stability.
- Effectively explain public social services programs and assist consumers with the application and follow up, to ensure services are received.
- Create consumer driven treatment plans in concert with consumers and doctor recommendations that reflect the needs of the medically fragile client.
- Monitor client’s progress and provide follow-up services.
- Maintain accurate documentation of service to clients. Enter accurate consumer date into HMIS in real time data, to reflect consumer activities and their progress.
- Maintain audit ready files at all times. (Random audit checks are regularly performed)
- Participate in multi-agency, multidisciplinary teams to report and review consumer progress.
- Other duties, as may be assigned.
Knowledge, Skills and Abilities:
- Knowledge of local social service agencies and programs as they pertain to the local homeless population.
- Knowledge of applicable statutes, ordinances, and codes.
- Knowledge of social service work, interviewing/referral techniques.
- Experience and understanding of homeless consumer issues.
- Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
- Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
- Effective written, verbal, and listening skills. Independently initiates and responds to correspondence.
- Ability to maintain order and exercise appropriate judgment in crisis situations.
- Is flexible, and can quickly adjust to shifting priorities.
- Establish and maintain cooperative working relationships with consumers, fellow employees, other agencies, and the general public.
- Organize work schedule, and budget time.
- Understand and follow all agency policies and procedures.
- General computer knowledge, and the ability to use Microsoft Office software.
- Ability to work flexible hours, as required by consumer case load.
- Ability to perform accurate mathematical equations to calculate client’s rent and utility costs, and request funding to pay those bills.
Qualifications, Education, and/or Equivalent Experience:
- Bachelor’s Degree in Social Work, or High School Diploma and (2) Two Years experience, substantially equivalent to a bachelor’s degree, or combination of education and experience.
- Professional experience, as required by education.
- Must pass background check and pre-employment drug test. Random drug screening is performed.
- Valid Nevada driver’s license, and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
