Intensive Case Manager
Hours: Monday - Thursday
Status: Full Time
Reports to: Rita Suites Program Manager
Purpose:
Under the supervision of the Rita Suites Program Manager, this position will provide direct case management services to homeless individuals and families who reside at the Rita Suites Non-Congregate Shelter
Duties, Functions and Responsibilities:
- Conduct initial interview/intake and client needs assessments.
- Provide direct support and arrange services for clients moving into and residing at the Rita Suites, to promote transition from homelessness to housing stability.
- Effectively explain mainstream programs and assist clients with accessing services; follow up to assure services are received.
- Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
- Create individualize case plans with the client to reflects their needs and goals.
- Meet with client regularly and assist with SOAR and/or other applications, and to address any other social barriers the client faces.
- Conduct weekly unit visits.
- Review and update Treatment Plan with client no less than bi-monthly.
- Continuously monitor clients’ progress and provide follow-up and wrap around services as needed.
- Enter case notes and data into HMIS daily in real time data, reflecting activities of the clients’ progress, and maintain audit-ready files at all times.
- Maintain clear boundaries with all clients and providers.
- Must maintain ethical standards of the agency at all times.
- Provide transportation to clients, to ensure that appointments are kept.
- Other duties, as may be assigned.
Knowledge, Skills and Abilities:
- Knowledge of local social service agencies and programs as they pertain to the local homeless population.
- Knowledge of applicable statutes, ordinances and codes.
- Knowledge of social service work, interviewing/referral techniques.
- Experience and understanding of homeless issues.
- Must be able to understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
- Must be able to communicate effectively with persons from a variety of social, cultural and economic backgrounds, and exhibit a demonstrated understanding and sensitivity to culturally diverse populations.
- Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
- Ability to maintain order, and exercise appropriate judgment in crisis situations.
- Must have the flexibility to quickly adjust to shifting priorities.
- Must be able to organize work schedule and budget time effectively.
- Understand and follow all agency policies and procedures.
- General computer knowledge and ability to use Microsoft Office software.
- Ability to work flexible hours, as required by consumer case load.
- Ability to perform accurate mathematical equations to assist client with budgeting.
Qualifications, Education and/or Experience:
- Bachelor’s Degree in Social Work, or any combination of education and experience substantially equivalent to a bachelor’s degree.
- Must pass a background check and pre-employment drug screening. Random drug screenings are performed.
- Must have a current NV driver’s license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
