Reports to: AHP Supervisor
Purpose:
Under the direct supervision of a AHP Supervisor, this position provides direct case management services to chronically homeless adults.
Duties, Functions, and Responsibilities:
- Conduct initial interview/intake, and client needs assessments.
- Provide support and arrange services to clients moving into long-term housing, to promote transition from homelessness to residential stability.
- Effectively explain public social services programs and assist clients with the application and follow up, to ensure services are received.
- Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
- Create client centered treatment plans that reflect the needs and desires of the client.
- Complete monthly Treatment Plan Reviews with the client, and obtain signatures.
- Monitor client’s progress and provide follow-up services.
- Enter case notes in true time data (daily) into HMIS, reflecting activities of the client’s progress.
- Maintain audit ready files at all times. (Random audit checks are regularly performed).
- Maintain clear boundaries with all clients and providers.
- Provide transportation to clients to ensure that all appointments are kept.
- Attend meetings and trainings, as required.
- Other duties, as may be assigned.
Knowledge, Skills, and Abilities:
- Knowledge of local social service agencies and programs, as they pertain to the local homeless population.
- Knowledge of applicable statutes, ordinances, and codes.
- Knowledge of social service work, and interviewing/referral techniques.
- Experience and understanding of homeless issues.
- Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
- Communicate effectively with persons from a variety of social, economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations.
- Effective written, verbal, and listening skills. Independently initiates and responds to correspondence.
- Ability to maintain order and exercise appropriate judgment in crisis situations.
- Is flexible and can quickly adjust to shifting priorities.
- Establish and maintain cooperative working relationships with clients, fellow employees, other agencies, and the general public.
- Organize work schedule, and budget time.
- Understand and follow all agency policies and procedures.
- General computer knowledge, and the ability to use Microsoft Office software.
- Ability to work flexible hours, as required by client case load.
- Ability to perform accurate mathematical equations, to calculate client’s rent and utility costs, and request funding to pay those bills.
Qualifications, Education, and/or Equivalent Experience:
- Bachelor’s Degree in Social Work or High School Diploma/GED with (2) two years of experience, substantially equivalent to a Bachelor’s Degree, or combination of education and experience.
- Experience working with the homeless population.
- Must be able to pass a background check and pre-employment drug screening. Random drug testing is performed.
- Must have a current NV driver’s license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
