Reports to: AHP Supervisor and Manager
Purpose:
Under the direct supervision of the AHP Supervisor, this position will provide direct case management services to chronically homeless and disabled families.
Duties, Functions, and Responsibilities:
- Conduct initial interview/intake and client needs assessments.
- Provide support and arrange services to clients moving into long term housing, to promote transition from homelessness to housing stability.
- Effectively explain mainstream programs and assist clients with accessing services; follow up to assure services are received.
- Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
- Create individualized case plans with the client to reflect their needs and goals.
- Meet with client monthly and complete income calculations and rent requests prior to the 1st of the following month.
- Conduct weekly/semi-monthly home visits.
- Review and update Treatment Plan with clients.
- Monitor clients’ progress and provide follow-up services.
- Enter case notes and data into HMIS daily, in real time data, reflecting the activities of the clients’ progress.
- Ensure that files are audit ready at all times. Audit checks are random.
- Maintain clear boundaries with all clients and providers.
- Must maintain ethical standards of the Agency at all times.
- Provide transportation to clients, to ensure that appointments are kept.
- Other duties, as may be assigned.
Knowledge, Skills, and Abilities:
- Knowledge of local social service agencies and programs as they pertain to the local homeless population.
- Knowledge of applicable statutes, ordinances, and codes.
- Knowledge of social service work, interviewing/referral techniques.
- Experience and understanding of homeless issues.
- Must be able to understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
- Must be able to communicate effectively with persons from a variety of social, cultural and economic backgrounds, and exhibit a demonstrated understanding and sensitivity to culturally diverse populations.
- Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
- Ability to maintain order, and exercise appropriate judgment in crisis situations.
- Must have the flexibility to quickly adjust to shifting priorities.
- Must be able to organize work schedule and budget time effectively.
- Understand and follow all agency policies and procedures.
- General computer knowledge and ability to use Microsoft Office software.
- Ability to work flexible hours, as required by consumer case load.
- Ability to perform accurate mathematical equations to calculate client rent and utility costs, and request funding to pay those bills.
Qualifications, Education, and/or Equivalent Experience:
- Bachelor’s degree in Social Work, or any combination of education and experience substantially equivalent to a bachelor’s degree.
- Must pass a background check and pre-employment drug screening. Random drug testing is performed.
- Must have a valid NV driver’s license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
