Reports to:         AHP Supervisor and Manager

 

 

Purpose:

Under the direct supervision of the AHP Supervisor, this position will provide direct case management services to chronically homeless and disabled families.

 

 

Duties, Functions, and Responsibilities:

  1. Conduct initial interview/intake and client needs assessments.
  2. Provide support and arrange services to clients moving into long term housing, to promote transition from homelessness to housing stability.
  3. Effectively explain mainstream programs and assist clients with accessing services; follow up to assure services are received.
  4. Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
  5. Create individualized case plans with the client to reflect their needs and goals.
  6. Meet with client monthly and complete income calculations and rent requests prior to the 1st of the following month.
  7. Conduct weekly/semi-monthly home visits.
  8. Review and update Treatment Plan with clients.
  9. Monitor clients’ progress and provide follow-up services.
  10. Enter case notes and data into HMIS daily, in real time data, reflecting the activities of the clients’ progress.
  11. Ensure that files are audit ready at all times.  Audit checks are random.
  12. Maintain clear boundaries with all clients and providers.
  13. Must maintain ethical standards of the Agency at all times.
  14. Provide transportation to clients, to ensure that appointments are kept.
  15. Other duties, as may be assigned.

 

Knowledge, Skills, and Abilities:

  1. Knowledge of local social service agencies and programs as they pertain to the local homeless population.
  2. Knowledge of applicable statutes, ordinances, and codes.
  3. Knowledge of social service work, interviewing/referral techniques.
  4. Experience and understanding of homeless issues.
  5. Must be able to understand and follow complex instructions and procedures.  Analyze situations accurately and adopt an effective course of action.
  6. Must be able to communicate effectively with persons from a variety of social, cultural and economic backgrounds, and exhibit a demonstrated understanding and sensitivity to culturally diverse populations.
  7. Effective written, verbal and listening skills.  Independently initiates and responds to correspondence.
  8. Ability to maintain order, and exercise appropriate judgment in crisis situations.
  9. Must have the flexibility to quickly adjust to shifting priorities.
  10. Must be able to organize work schedule and budget time effectively.
  11. Understand and follow all agency policies and procedures.
  12. General computer knowledge and ability to use Microsoft Office software.
  13. Ability to work flexible hours, as required by consumer case load.
  14. Ability to perform accurate mathematical equations to calculate client rent and utility costs, and request funding to pay those bills.

 

Qualifications, Education, and/or Equivalent Experience:

  1. Bachelor’s degree in Social Work, or any combination of education and experience substantially equivalent to a bachelor’s degree.
  2. Must pass a background check and pre-employment drug screening.  Random drug testing is performed.
  3. Must have a valid NV driver’s license and clean DMV record.  Driver History Reports are required to be submitted on a quarterly basis.
